How Aftercare Works at Broadview

Aftercare planning is part of treatment, not a conversation that happens at the end. From admission, our case management team is mapping the next level of care: PHP, IOP, MAT continuation, sober-living placement, outpatient therapy, family support, and follow-up medical care. Specific clinics. Specific providers. Specific first-appointment dates.

We don’t run PHP or IOP in-house. We are a 12-bed detox and residential program — that’s our scope. But our case management team has worked with most major step-down programs in San Diego and beyond. We don’t just give you a list. We make the warm handoff, schedule the first appointment, and coordinate the medical record transfer before you walk out the door.

Why This Matters

The most dangerous moment in recovery is the first 30 days after residential. Programs that send patients home with a folder and a phone number see relapse rates that match what the research predicts. Programs that schedule the first appointment, confirm the insurance, and put a name and a face to the next provider — those see different outcomes. We are firmly in the second camp.

What Our Case Management Team Coordinates

PHP (Partial Hospitalization)

Six hours of structured outpatient programming, 5-6 days a week. Best for patients who need significant support without 24/7 supervision.

IOP (Intensive Outpatient)

Three hours of group + individual therapy, 3-5 days a week. The most common step-down from residential.

MAT Continuation

Suboxone, Sublocade, or Vivitrol prescriptions transferred to an outpatient provider with no gap in coverage.

Sober Living Placement

We connect patients with vetted sober-living homes appropriate to their recovery stage and personal situation.

Outpatient Therapy

Individual therapy continuation with a therapist matched to the patient’s clinical needs.

Family Support Resources

Family education, Al-Anon meetings, family therapy referrals, and ongoing communication channels.
